Sanford receives excellence award

Sanford Luverne Clinic was recently recognized with a 2016 Minnesota Bridges to Excellence Award for delivering exceptional quality outcomes for patients with diabetes, vascular disease and  depression.
The Minnesota Bridges to Excellence program is a health care purchaser-led pay for performance program that recognizes improvements in patient care in clinics across the state.
The program, operated by the Minnesota Health Action Group, honored 276 Minnesota and border state clinics this year that have improved patient outcomes by at least 10 percentage points in the last year.
Sanford Luverne Clinic was selected for the excellence award through its transformation of the model of treatment for diabetes, vascular disease and depression management, including an expanded care coordination team. 
The care team includes a physician, nurse, RN health coach, behavioral health triage therapist, dietitian, care coordination assistant and most importantly the patient. 
Regular patient follow-up communication is vital to the improved health of the patient, and the care coordination assistant works closely with providers, staff and patients to ensure that lab tests routinely coincide with office visits. 
“We have been able to provide patients diagnosed with diabetes, vascular disease and depression greater care coordination services, building on the excellent care available at Sanford Luverne and improving the integrated care in response to Luverne area health needs,” said Laurie Jensen, director of clinic operations at Sanford Luverne Clinic.
“This team approach demonstrates our understanding that the improvement of patient outcomes equals the improvement of their quality of life.”
Established by the Minnesota Health Action Group in 2006, the Minnesota Bridges to Excellence program uses publicly reported outcomes data to identify clinics that meet or exceed optimal care standards for a specified percentage of patients with diabetes and vascular disease.
